Your first appointment is typically an hour long. We request that you arrive to your appointment 20-30 minutes early to complete the New Patient paperwork and verify your insurance. You will receive a full assessment including review of your health history and condition(s). After completion of the appointment the provider will inform you about next steps. Most of the patients we see are in some degree of pain, therefore we strive to get you started on a treatment plan as soon as possible. Before leaving, you will be scheduled for your next appointment, usually occurring later that same week or early the next week.

What to bring to your first visit:

• Insurance Card
• Photo ID
• Medical records if possible, including recent imaging reports
• List of medications
• Your planner/agenda for scheduling additional appointments
• If affiliated with a claim case such as Motor Vehicle Accident or Workmen’s Compensation, bring the following information: Claim number, insurance information, adjustor name and contact information, and details of the injury/accident.
